Director of Memory Care

Kingsley Shores is seeking a full-time Director of Memory Care

The purpose of the Director of Memory Care is to be the primary program developer/implementer and customer service representative to residents and families who live in the Reflections Neighborhoods. This position is responsible for: the success of the Reflections Neighborhoods; the means of providing meaningful and excellent programs for residents with dementia; linking residents and families with other services; problem solving with the rest of the team on campus; and to do internal, and some external, marketing- thus preventing move outs as much as possible, as well as assessing move ins with the Director of Marketing. Working closely and daily with the Director of Clinical Services to best manage the overall nursing staff, to facilitate assessments and moves from AL to Memory Care, and to communicate with the RN Clinical Nurse, as well as communication with the Director of Programs and others is critical in understanding resident needs and abilities. Initial and monthly training of staff is also critical- especially the direct health care staff.

Job Requirements:

Education: Minimum a high school diploma; bachelor's degree preferred. Other education: Alzheimer's and Dementia training, case management services, train the trainer sessions, and marking is helpful

Experience: At least 2 years in geriatrics, and/or activities related position, with 1 year of that in Alzheimer or other Dementia- related program developments and implementation services position. Minimum of two years supervision experience.

Activities Team Coach -Full Time

  • Orchestrates and leads activities for resident participation with individuals and in small and large groups.
  • Conducts resident activity interest surveys and assessments.  Attends care conferences and completes progress notes.
  • Establishes one-on-one relationships with residents. Maintains visit schedule per care plans.

  • Provides hands-on/on-the-job training to newly hired activities assistants. Provides skill-set retraining as assigned by the health center Lifestyle Lead.

  • Maintains inventory par levels and track inventory for supplies.
  • Complies with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
  • Obtains knowledge of and demonstrates the principles of Living Lifespace, person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, team work and quality.
  • Team member will support, actively participate and act in accordance with the principles of the Living Lifespace service culture and be familiar with and fluent in the language and service pillars of Living Lifespace.

  • Prefer associates or bachelor's degree.
  • Two years of experience in recreational and/or senior living programs.
  • Familiarity with current health center regulatory requirements for activities.
  • Related certifications and training appreciated.

Memory Care Program Coordinator- Part Time

Job Description:

Parkshore Senior Campus in St Louis Park is seeking a Part-Time Memory Care Program Coordinator in our Reflections Neighborhood. The position helps the Director of Reflections in planning, coordinating and implementing engagement and programming for our residents to meet their individual and group needs.

This position is part-time, two days a week, Fridays and Saturdays 9:00 a.m.-4:00 p.m. Additional hours may be requested based on campus activities. 

In this role, the Reflections Program Coordinator will:
* Plan and lead activities as assigned by the Director of Reflections.
* Invite residents to activities and push resident wheelchairs when needed.
* Maintain contact with room-bound residents to provide one-on-one programming.
* Attend and participate in departmental staff meetings, online training, and in-service training sessions.
* Assist with resident bus outings.
* Organize, clean and maintain activity supplies and storage areas as needed.
* Complete additional tasks as needed.

Director of Activities- Full Time

Job Summary: This position is responsible for planning, developing and providing a quality Activity /Social Program designed to assist residents in using their personal potential both physically and mentally

Essential Functions:

  • Must have excellent communication skills; written and verbal: to be able to read aloud, speak in front of groups and formulate ideas on paper
  • Must have excellent organizational skills
  • Must possess the following mental abilities: alertness, judgement, initiative, mathematical ability, creativity and imagination
  • Must be able to operate a motor vehicle

Responsibilities, Duties:
  • Develops and maintains an activities program which is reflective of the residents interest. Leads activity groups including exercise classes
  • Communicates and coordinates activity programming with all departments
  • Supervises the activities programming provided by all personnel
  • Developing and updating activities' policies and procedures
  • Responsible to meet requirement of Federal and State Health Department's and other regulatory entities as they relate to activities
  • Must order and maintain supplies needed for activities
  • Assists other departments with special events and family functions
  • Ability to learn and assist in other areas of the facility
  • Report resident changes to the staff on duty
  • Documents as required
  • Offers suggestions and ideas for improvement
  • Responds appropriately to safety hazards, fire drills and other emergency situations
  • Reports injuries to self, residents or visitors immediately to the supervisor
  • Maintains at all times privacy and confidentially of resident, resident's records and resident's living environment
  • Understands and adheres to the Vulnerable Adult and Abuse Policy. Is familiar with the Resident's Bill of Rights and adheres to it
  • Appropriately communicates with resident and/or resident's family
  • Assist with the transportation of residents to and from local appointments/and/or doctor's appointment. (If required)
  • Must work as a team member with other personnel, supervisor, RN and the community
